Arkansas Medicaid doula coverage: status and next steps

Arkansas has enacted Medicaid doula coverage, but enrollment and billing were not open when this guide was checked on July 25, 2026. The state missed its December 2025 and June 2026 deadlines, and DHS had not confirmed the reported August 1 target. Community-based doula certification and the NPI application are open now. Wait for DHS to publish final enrollment and billing instructions before assuming a service will be payable.

Proposed payment rates

Planned visit and delivery rates; billing is not open yet

• Planned rate: $64.45 per prenatal or postpartum visit, with up to 6 visits per pregnancy. DHS retained this amount through the final rule, but it was not yet in a published fee schedule
• Planned delivery-attendance rates: $468.55 for vaginal delivery, $325.45 for cesarean delivery, $527.78 for vaginal delivery after a prior cesarean, and $546.50 for cesarean delivery after a vaginal attempt
• The planned maximum is $933.20 for 6 visits plus the largest listed delivery fee. Do not treat it as billable until DHS opens enrollment and publishes the fee schedule
• Doula-specific billing codes and the final fee schedule were not published as of July 25, 2026. Check the DHS fee schedule when enrollment opens

Six visits per pregnancy used flexibly across prenatal and postpartum care, with postpartum visits completed within 16 weeks, plus in-person attendance at a hospital delivery. Visits are at least 60 minutes; the first is in person and at least one takes place in the client's home. Telehealth is allowed after the first visit for prenatal or postpartum care. Home-birth attendance is not covered.

Who is expected to qualify

• 18 or older, with a free Type 1 NPI and the state-required taxonomy code
• Arkansas Department of Health community-based doula certification (Act 965 of 2025): training pathway (approved-organization training with at least 16 hours didactic plus support at 3 births) or experiential pathway (5+ years of doula experience within the past 7, with testimonial letters); automatic and reciprocal routes also exist
• Certification is valid 2 years and renews with 10 hours of professional development
• The ADH certification application fee is $50. The Medicaid materials reviewed do not add a separate doula enrollment fee
• The member must have the standing recommendation adopted by DHS. Members in Life360 HOME and members already receiving community health worker services are excluded from this doula benefit
• Per-encounter documentation requirements apply (dates, times, service description, birth plan and social-needs assessment results). Records are subject to post-payment audit

Current and planned steps

1. Step 1

   Get ADH-certified now, this step is open today

   Apply through the Arkansas Department of Health's Community-Based Doula Certification program. It offers Pathway A, Pathway B, automatic, and reciprocal applications. The 2026 approved-training list names 13 organizations. The Doula Alliance of Arkansas can help with the certification process.

2. Step 2

   Get your free Type 1 NPI

   Apply through NPPES with taxonomy 374J00000X. It costs nothing and is required by the Medicaid manual.

3. Step 3

   Enroll as an Arkansas Medicaid provider when enrollment opens

   Enrollment is expected to run through the Arkansas Medicaid provider portal (portal.mmis.arkansas.gov), using an online application, disclosures, and the provider contract. A doula-specific enrollment type was not visible as of July 25, 2026. Check the portal and DHS program materials for an opening announcement.

4. Step 4

   Bill fee-for-service once live

   Doulas will bill Arkansas Medicaid directly on the CMS-1500 claim form. No prior authorization is planned for the standard benefit. DHS replaced an individual referral requirement with a standing recommendation from its medical director.

How payment is expected to work

The standard benefit is designed for fee-for-service billing to the state's fiscal agent rather than separate plan contracts. Wait for DHS to publish the final provider-enrollment and claims instructions before assuming a service will be payable.

What to plan for

Arkansas missed its December 2025 and June 2026 launch deadlines. As of July 25, 2026, DHS had not published the doula billing codes, final fee schedule, or enrollment type. Do not provide services on the assumption that Medicaid will pay until DHS confirms enrollment and claims instructions.
