How to enroll as a Medicaid doula in Oklahoma

SoonerCare has covered doula services since July 1, 2023. At the July 1, 2026 fee schedule, eight prenatal or postpartum visits plus birth attendance pay between $834.46 and $1,033.18, depending on the delivery route. Enrollment requires an accepted certification, a Type 1 NPI, and an OHCA provider contract. Availity lets you submit credentialing information to all three SoonerSelect plans, but you still need a contract with each plan you intend to bill.
What Medicaid pays

- Each prenatal or postpartum visit pays $67.812 at the July 1, 2026 schedule. Eight visits total $542.50 after rounding
- Birth attendance pays 40% of the physician delivery-only rate for a cesarean and 65% for a vaginal birth, VBAC, or cesarean following a vaginal delivery attempt
- Using the July 1, 2026 physician schedule, eight visits plus birth total $834.46 for a cesarean, $959.36 for a vaginal birth, $1,016.44 for a VBAC, or $1,033.18 for a cesarean following a vaginal delivery attempt
8 prenatal/postpartum visits (each at least 60 minutes; telehealth allowed) plus one in-person labor & delivery visit per pregnancy; home-birth attendance is not covered; prior authorization can extend visits for medical circumstances.
Who can apply
- 18+, certified by an OHCA-recognized doula certifying organization (birth, postpartum, full-spectrum, or community-based certification all accepted)
- The recognized organization must verify CPR certification, require 2 professional references (at least one from a client), and recertify at least every 3 years
- Oklahoma-based recognized trainers include Imani's Village (OKC) and the Oklahoma Birth Equity Institute
- A physician or other authorized licensed practitioner must recommend services, and the SoonerCare Referral Form must be submitted

Submit credentialing information, then contract with each plan
Availity lets Medicaid-only providers send one credentialing application to Aetna, Humana Healthy Horizons, Oklahoma Complete Health, or all three. Each plan still makes its own decision and requires its own contract.

Availity centralizes credentialing information for Aetna, Humana Healthy Horizons, and Oklahoma Complete Health. It does not replace plan contracting. Select the plans you want to join, then complete each plan's contract and confirm your effective date before serving its members.
What to plan for
Every client needs a completed referral form. OHCA enrollment and Availity credentialing are separate, and a doula must contract with each SoonerSelect plan they intend to bill. Payment also varies with the delivery route.
