How to enroll as a Medicaid doula in Utah

Utah's Medicaid doula benefit took effect April 1, 2026 under SB 284. You can qualify through an approved certification and birth experience or through the experience pathway. Enrollment runs through PRISM. The benefit covers 16 prenatal units, 12 postpartum units, and one flat-rate birth attendance. Utah publishes current dollar amounts only in its Coverage and Reimbursement Lookup tool, so check T1032 and T1033 there before billing.
What Medicaid pays
Rates set spring 2026. Check Utah's Coverage & Reimbursement Lookup for T1032/T1033
- Structure: 16 units (240 minutes) of prenatal T1032 + 12 units (180 minutes) of postpartum T1032 + one flat-rate T1033 birth attendance per pregnancy, with prior authorization available for more
- Utah publishes rates only through its interactive Coverage & Reimbursement Lookup tool. No static fee schedule exists, so verify the current unit rates there
- Coverage includes pregnancies ending in miscarriage or stillbirth; claims within 12 months of service
16 prenatal units + 12 postpartum units of 15-minute T1032 time + one flat-rate birth attendance per pregnancy, with additional units available by prior authorization.
Who can apply
- Certification pathway: training from one of the 14 organizations on Utah Medicaid's current approved list, plus at least 3 births attended in the past 5 years, current CPR, and the Doula Code of Conduct
- Experience pathway: 10+ births attended (5 in the past 2 years), 4 recommendation letters (2 from clinical birth-team members), CPR, and signed attestations. No certification required
- No attestation fee; keep your supporting documentation, which the state can request

Complete the Doula Provider Attestation
Choose the certification or experience pathway and use the current attestation from the Utah Medicaid Women's Services page. Keep the records that support each answer.

Enroll through PRISM
Apply under Utah's doula provider type and respond to any request for missing information. Processing time depends on whether the application is complete.

Contact the accountable care organizations you plan to bill
Utah Medicaid physical health is delivered through Healthy U, Select Health Community Care, Molina, and Health Choice Utah. Ask each organization for its current contracting and billing instructions.
How you get paid
PRISM enrollment does not replace an accountable care organization's contracting process. Confirm the member's organization, then ask that organization for its current doula application, payment policy, and billing instructions.
What to plan for
The benefit began in April 2026. Rates are available only through the interactive lookup tool, plan onboarding processes differ, and the attestation form has already been revised. Download the current forms and plan guidance rather than relying on saved copies.
