NPI application checklist

The NPI application is free, and CMS estimates that the online form takes about 20 minutes. Processing time varies. Print this page and use it with the NPI guide.

Before you start

  • ☐ Your legal name, date of birth, email, and phone number
  • ☐ Your SSN, or an ITIN if you do not qualify for an SSN. Do not use an EIN for a Type 1 application
  • ☐ If you have neither an SSN nor ITIN, follow the identity-document instructions on the current CMS form
  • ☐ A business mailing address and practice-location address. Both will be public in the NPI Registry
  • ☐ Check the address rules: a mailing address may be a PO box; the practice-location field permits one only for an individual provider who has no physical location other than home

The application

  • ☐ Go to nppes.cms.hhs.gov, the official CMS application site
  • ☐ Create an Identity & Access (I&A) account
  • ☐ Apply for a Type 1 (Individual) NPI
  • ☐ Enter your SSN, or your ITIN if you do not qualify for an SSN
  • ☐ Enter both required addresses and review what will appear publicly
  • ☐ Taxonomy code: search "doula," select 374J00000X — Doula
  • ☐ Review your legal name, identity information, addresses, and taxonomy
  • ☐ Submit and save the confirmation. CMS does not promise a fixed approval time

When the number arrives

  • ☐ Save the enumeration notice with your business records
  • ☐ Search the NPI Registry and correct any errors through NPPES
  • ☐ Add the NPI to an itemized receipt only when a program asks for it
  • ☐ If you want to become a Medicaid doula, complete the separate state enrollment and contracting steps in the state guides
  • ☐ Update NPPES when your name, address, taxonomy, or other record details change

Remember

  • An NPI does not license or credential you, enroll you with a payer, or guarantee reimbursement
  • Type 1 identifies a person; Type 2 identifies an eligible organization
  • A Type 2 organization uses its legal business name and EIN
  • CMS charges no application fee. Paid application help is optional

The current release uses payment-record dates and generic descriptions, so check the document against the recipient's requirements before sharing it.
